Actionable lessons, not observations
Communication could be better is an observation. Agree a single written decision route with the supplier before contract signature is a lesson. The test is whether someone could act on it next time without asking you what you meant.
Blame free language
Describe the condition that produced the outcome rather than the person who was standing nearest. Logs that name people stop being honest within one project.
