Fields that make a decision reusable
The decision itself, the context that forced it, the options considered, the rationale and trade off, the accountable decision maker, anyone consulted, a status of proposed, decided, superseded or revisit, and any follow up action.
A decision log is not meeting minutes
Minutes record that a conversation happened. A decision log records the reasoning, so that in six months someone can tell whether the decision is still right or whether the ground has moved.
The common failure
Recording the decision but not the reason. Without the rationale and the options rejected, the log is a list of instructions nobody can safely revisit.
